  • Fill in the sections asking for member name, dependent (if applicable), and signature only.

  • In the signature section, "employee" refers to the member and "employer" refers to Centric.

  • Monthly payments will be processed as a recurring Centric subscription charge.

  • Cards are typically available within 1–3 weeks after enrollment and can be used for eligible services immediately where no waiting period applies.

  • Members enrolled before June 15th will not be required to complete a medical for enrollment.

Comprehensive memberships allow additional dependents to be added for an extra monthly cost. Dependents may include a spouse or children up to age 27.

Membership cards are typically made available within 1–3 weeks after enrollment and can be used to access eligible services.

Memberships are billed monthly through recurring subscription payments processed by Centric.

If enrolled before June 15th, no medical examination will be required during enrollment.

Pilates sessions included within memberships can be booked through Centric Pilates based on availability.

Coverage details, waiting periods, and available services may vary depending on the selected membership.
